Parish Registration Form

To become a parishioner fill out the registration form and turn it in to the Sunday collection basket or drop it off during business hours at the Parish Office. 

Please complete the forms as thoroughly as possible. If information is missing, someone from the office will call to gather the missing information. After joining, you will begin receiving donation envelopes in the mail. Please turn an envelope in each weekend, even if you cannot donate, as we use the envelope system for attendance purposes as well as for a financial record.
